Dear Portfolio Committee on Home Affairs,

I have been trying to get an amended Death Certificatesince October 2019 and applied for this at Standerton DHA. I have tried to follow up on his with the DHA call centre and DHA staff and despite repeated promises regarding return calls and expediting the issue - nothing has happened

I have a copy of the original applicattion available and will forward this if required.

In the absence of this amended document I cannot register or finalise the estate and the delay is not helping. Attempts to appeal for assistance through the Standerton office fall on deaf ears.

Please can you assist
